Provider Referral & Discharge

Our Center is a network hub of health and human service agencies, including three homeless shelters (The Life House Cottage, Bethel Day Shelter, and The Haven Men’s Shelter) that provide free programs, resources, and services to impoverished men, women, and families. Each on-site agency operates independently with its own staff, resources, rules, and operational processes. We collaborate at the Community Center to coordinate care, working together to help individuals achieve stability and self-sufficiency while ensuring their dignity is upheld.


Important Ordinance Update- Coordination Requirement: In response to the City of Rock Hill's ordinance, effective February 2024, all transports of at-risk individuals to health and human service agencies must be properly coordinated. This ordinance ensures that necessary resources and beds are available upon the individual’s arrival to ensure a warm-handoff of care. Non-compliance may lead to significant fines by the City of Rock Hill Police Department. Full ordinance details are available here.
