Pathways Community Center has taken a significant step towards accountability and data transparency by partnering with the Unite Us software. This integration of Unite Us ensures that every client who comes to Pathways receives attentive coordination of care and follow-up to meet their diverse and changing needs. This approach assists case managers and social workers in identifying which professionals are supporting each client, thereby preventing overlap in service provision.The monthly collective impact report clearly explains our united efforts in guiding clients toward self-sufficiency. Highlighting everything from educational achievements to employment successes, housing stability, and more, the report demonstrates the positive change we've achieved together, strengthening our community's socio-economic fabric.
