COORDINATED CARE

Through our partners on-site at Pathways, coordinated care includes food, clothing, shelter and housing options, counseling, spiritual guidance, governmental assistance, medical and mental health care, substance abuse treatment, education, transportation, life skills and job training, and employment placement.

Making these services accessible at a single location is what makes Pathways so unique.  Rather than directing people needing assistance to first one agency, then another, all in different locations, Pathways provides a multi-functional community center where agencies unite to accomplish one goal - to renew the lives of people in need.
